Does anyone have the link for the application?  I want to get my packet out today but just need the page for Commitment 5 to get signatures.

Have you visited the Playful City USA website (www.playfulcityusa.org or Playful City USA under the Advocate heading on the navigation bar) yet and clicked "Request an application today!" - this requires you to finish a short survey and then download a pdf version of our application.
