I am wrapping up our Playful city packet to meet the April 1st deadline but I do not have the application.
I have requested it to be emailed to me a few times but think we may be having issues with it being caught in our SPAM yet I check there and cannot find it. Ben or if anyone else has a copy could you please email me an application to this address [removed by admin] . I believe I have everything else but that application. THANK YOU! - Shannon

Have you visited the Playful City USA website (www.playfulcityusa.org or Playful City USA under the Advocate heading on the navigation bar) yet and clicked "Request an application today!" - this requires you to finish a short survey and then download a pdf version of our application.
